Tuberculosis (TB) is the leading cause of death and ill-health from an infectious disease agent worldwide. The gap between TB case
notifications and estimated TB incidence indicates that TB case finding is still missing many TB cases. Studies have found that TB contacts have an extraordinary high TB burden. Complicated algorithms, and reliance on poorly performing symptom screening tools, have impeded diagnosis and led to missed opportunities for TB prevention among TB contacts in routine programs. The Lancet Commission on TB called for bold and innovative implementation strategies to promptly identify people with active disease in such high-risk populations, and to increase access to TB preventive therapy (TPT). CUT-TB aims to evaluate a Test-and-Treat strategy that includes universal TB testing of all household contacts regardless of symptoms, complemented by treatment of either TB disease or l atent TB infection (LTBI). Our multidisciplinary team of African, American and European researchers will evaluate this Test-and-Treat strategy in three countries including South Africa, Tanzania and Lesotho.
