Primary Health Care Coordinator

Reporting: Health Reform Program Lead

Duty station: Butha Buthe District Health Management Team (DHMT)

Responsibilities include but are not limited to:

– To provide continuous support to the District Health Management team in provision of high quality primary health care led service delivery in the district.

– Provide technical support, mentorship and supportive supervision to health facilities jointly with DHMTS.

– ldentify gaps in health service delivery and plan interventions with DHMTS and health centres.

– Work closely with the health centre nurse in charge to improve service delivery in the communities.

– Follow up on action plan implementation in health centres.

– Supervise Village Health Worker Coordinators and follow up on village health workers’ activities.

– Participate in meetings which are organized by DHMTs to improve the health system in the district.

– Work closely with PIH data records and Information office at DHMTS to ensure proper, timely and accurate data collection and submission from the health facilities.

– To coordinate the overall PlH support in the district.

– To provide weekly and monthly work progress reports.

Minimum Requirements:

– General Nursing and Midwifery with 5 years of professional experience preferably in technical field.

– Post graduate qualification in Primary Health Care will be an added advantage

– Ability to work under pressure and meet deadlines, manage multiple assignments and prioritise workload.

– Excellent communication (verbal and written) and interpersonal skills.

– Proficiency in Microsoft Office.

– Commitment to Prevention of Sexual Exploitation, Abuse and Sexual Harassment
