Background
Our client is implementing a comprehensive Primary Health Care (PHC) model in Leribe District, aimed at improving care for patients with complex, multimorbid conditions. The initiative focuses on chronic diseases such as diabetes, hypertension, epilepsy, and mental health challenges including depression and anxiety. The facility centralizes services to enhance outcomes and wellness, integrating quality mental health care to improve both physical and mental health.
Key Responsibilities
- Deliver high-quality, patient-centered nursing care
- Conduct patient assessments and maintain accurate records
- Support quality improvement and data reporting initiatives
- Lead community outreach and stakeholder engagement
- Supervise staff and coordinate clinic operations
- Use research findings to improve health service delivery
- Disseminate evidence-based practices to other health facilities and the District Health Management Team (DHMT)
Qualifications
- Education: Bachelor’s Degree in Nursing
- Registration: Valid registration with the Lesotho Nursing Council
Skills, Knowledge & Attributes
- Minimum of 3 years’ clinical experience (preferably in chronic disease care)
- Computer literacy (MS Word, Excel, PowerPoint)
- Excellent clinical judgment and problem-solving skills
- Strong communication and teamwork abilities
- High level of organization and attention to detail
- Strong leadership and data management skills
- Knowledge of NCD protocols and quality improvement tools
