Position: Linkage and Retention Assistant
Duty Station: LDF Health Centre – Maseru

AHF is seeking a dynamic, and results-oriented Linkage and Retention Assistant.

AHF launched its international operations in Africa in 2002. Today, the Africa Bureau supports high-impact HIV/AIDS programs in 15 countries namely Eswatini, Ethiopia, Ghana, Kenya, Lesotho, Malawi, Mozambique, Nigeria, Rwanda, Sierra Leone, South Africa, Tanzania, Uganda, Zambia and Zimbabwe.

POSITION SUMMARY

The Linkage and Retention Assistant supports the implementation of HIV care and treatment services by ensuring timely linkage of clients to care, improving retention in treatment programs, and supporting adherence to antiretroviral therapy (ART). The incumbent works closely with healthcare providers, community and village health workers, and facility staff to track clients across the HIV care continuum and contribute to achieving program targets.

KEY RESPONSIBILITIES

The successful candidate will:

  • Facilitate timely enrollment of newly diagnosed HIV-positive clients into HIV care and treatment services.
  • Track referrals from testing points to treatment sites and ensure successful linkage and engagement in medical care.
  • Follow up on clients who have not enrolled in care within the prescribed timeframe through phone calls and arranged home visits.
  • Update track position to confirm linkage of clients to care.
  • Document linkage and navigation data from client encounters.
  • Maintain accurate linkage records and registers.
  • Identify clients at risk of treatment interruption and provide appropriate follow-up.
  • Support implementation of retention strategies, including appointment reminders and follow-up tracking.
  • Assist in monitoring missed appointments and lost-to-follow-up cases.
  • Support the team to trace, contact and bring back those clients that are urgently required to return to the clinic, those with suppressed viral load, positive TB test results etc.
  • Identify and document barriers clients experienced in accessing HIV care, treatment and other services, and work with the client using a strength-based approach to identify solutions and overcome barriers.
  • Support clients on care and treatment through regular engagement to ensure adherence to medication.
  • Reach out to partners of clients that recently tested HIV positive and invite them to the clinic for HIV testing and counselling services.
  • Reinforce treatment goals that clients develop with medical care providers during one-on-one or group counselling sessions.
  • Provide adherence counselling and treatment literacy education as assigned.
  • Support differentiated service delivery models where applicable.
  • Collect, verify, and enter linkage and retention data accurately and timely.
  • Update patient tracking tools, registers, and databases and share with relevant staff for utilisation.
  • Prepare weekly, monthly, and quarterly reports on linkage and retention indicators.
  • Ensure data quality through routine proper documentation, verification and validation exercises.
  • Collaborate with community healthcare workers, peer educators, and community-based organizations to reach newly diagnosed HIV positive clients that haven’t been linked to care and treatment.
  • In conjunction with Community tracers, conduct community follow-up of clients that cannot be reached on phone.
  • Assist with liaising clients and community-based case management providers once they are successfully re-engaged in medical care.
  • Participate in multidisciplinary meetings to review client outcomes.
  • Support community outreach activities aimed at improving retention in care.
  • Maintain confidentiality and ethical standards in handling client information.
  • Participate in continuous quality improvement activities.
  • Assist in identifying gaps in linkage and retention services and recommend corrective actions.

THE IDEAL CANDIDATE SHOULD POSSESS

  • Must possess Diploma or Degree in Nursing, Public Health, Social Work, Counselling, Health Sciences, or a related field.
  • A minimum of 2 years’ experience working with clients in public health institutions, community services or medical setting.
  • At least 1–2 years of experience in HIV/AIDS programs, public health, or community health services.
  • Experience working with HIV care and treatment programs is an added advantage.
  • Experience in data collection, reporting, and client tracking systems.
  • Should have strong interpersonal and communication skills both verbal and written.
  • Ability to function under dynamic circumstances and proficiency in computer skills.
  • Have undergone at least a short course training in HIV Care and management.
