Country Context:

The endTB clinical trial is a phase III randomized controlled trial of five novel, all oral, 9-month regimens compared to the current standard of care for MDR-TB. The trial will combine the first new TB drugs developed in almost 50 years — bedaquiline and delamanid —with other oral TB drugs such as clofazimine, linezolid, fluoroquinolones and pyrazinamide into experimental new treatments. The clinical trial is expected to enrol 750 patients across six countries: Georgia, Kazakhstan, Kyrgyzstan, Lesotho, Peru and South Africa. These are all countries with significant TB burdens, where MSF or Partners in Health support local MDR-TB treatment activities. At the South African site, the trial will take place at four MSF-supported clinics in Khayelitsha, where it aims to enrol 100 participants with a follow up period of two years.

SUMMARY OF POSITION: MSF seeks to employ a suitable qualified Medical Doctor to join the endTB trial team in Khayelitsha. The purpose of the post is to provide quality medical care to trial participants, and to conduct trial procedures in accordance adapted and updated medical knowledge, MSF protocols, endTB Trial Protocol and procedures and Good Clinical Practice (GCP).

REQUIREMENTS

· A medical degree/ MBChB degree

· Current registration with the Health Professions Council of South Africa (HPCSA)

· South African citizenship or a valid work visa

· At least 6-months experience in managing patients with HIV-associated TB and MDR-TB in a primary care context (essential)

· Previous clinical trial or research experience (advantageous)

· GCP training (advantageous)

· Word, Excel, Powerpoint and all other basic office software and the use of online databases (essential)

· Excellent organizational, recording and reporting

· Ability to analyze and interpret data and write written reports with attention to details

· Strong interest in the care and treatment of people with RR-TB

· Strong interpersonal and communication skills and the ability supervise and work in a team

· Prior experience with MSF or similar NGO will be an added advantage

· Languages: English (fluent, spoken / written) isiXhosa (advantageous)

· South African citizenship or a valid work visa is a must

MAIN RESPONSIBILITIES

· Undergo training in the protocol, trial procedures, and GCP (if needed).

· Engage with clinic staff, and assist endTB trial staff and MSF counsellors to identify and recruit potentially eligible rifampicin-resistant TB patients for the trial.

· Together with a research nurse, perform screening procedures for potential participants to assess eligibility for the trial.

· Together with a research nurse, perform trial follow up visits and associated procedures in accordance with the protocol and GCP.

· Prescribe study drugs, ancillary drugs, and antiretroviral agents to trial participants.

· Perform medical assessments at every trial visit (and at unscheduled visits) to monitor clinical progress and to assess for adverse events.

· Perform medical assessments and follow up visits of trial participants in hospital when necessary.

· Assist research nurses with trial procedures, including phlebotomy, ECGs, toxicity screening.

· Complete Case Report Forms and other trial documents.

· Discuss clinical problems with the site Principal Investigator, and prepare reports for the trial Clinical Advisory Committee where appropriate.

· Engage with clinic staff and other members of the endTB trial team to identify medical problems or difficulty with adherence to study drugs.

· Liaise with clinic staff, including doctors, nurses, and counsellors, to share and discuss clinical progress of trial participants.

· Maintain regular contact with clinic doctors regarding the recruitment and management of trial participants. This includes writing and responding to referral letters where required.

· Travel to trial clinic sites in Khayelitsha.

· Compile reports on serious adverse events, and communicate these to the trial pharmacovigilance unit and local regulatory authorities (together with the Trial Coordinator and site Principal Investigator).

· Assist with quality control and data entry and management.

· Attend regular meetings with the endTB trial team.

· Attend meetings related to the endTB trial, including at MSF, clinical, academic, and community forums.

· Supervise and support, in close coordination with the HR department, the associated processes (training, performance evaluation, development and internal communication) of the the trial nurse and counselor, under his/her responsibility.

· Contribute to advocacy activities within the MSF endTB team, and Khayelitsha project as a whole.
