Position Title: Data Clerk

Project: GAIN
Reports To: M&E Officer


Position Summary

The Data Clerk Officer will be responsible for ensuring accurate, complete, timely, and confidential collection, capturing, verification, analysis, and reporting of health data at Paray Mission Hospital under the GAIN Project. The role supports strengthened Monitoring and Evaluation (M&E) systems to inform clinical decision‑making, program performance tracking, and reporting to stakeholders.


Key Responsibilities

M&E System Strengthening
  • Review and verify clinical registers, tally sheets, and reporting tools to ensure accurate, complete, and consistent documentation by clinicians
  • Conduct routine data quality checks (completeness, accuracy, and consistency) across all prescribed registers and tools
  • Ensure proper organisation, safekeeping, and archiving of hospital records, including active and archived registers, reports, and daily tally sheets
  • Maintain strict confidentiality of patient, hospital, and project‑related data in line with ethical and legal standards
  • Ensure continuous availability and appropriate use of prescribed registers, tally sheets, and reporting forms
  • Support implementation and adherence to national and project‑specific M&E guidelines and tools
Data Capturing, Verification, Analysis, and Reporting
  • Capture and update individual patient‑level data into CommCare accurately on a daily basis
  • Generate, compile, and validate daily, weekly, and monthly reports in collaboration with clinicians and supervisors
  • Ensure timely submission of complete and accurate reports to PIH Central Office, in hard copy and electronic formats
  • Retrieve and consolidate data from all available data systems for reporting, audits, and decision‑making
  • Conduct basic data analysis to identify trends, gaps, and inconsistencies, and support interpretation of findings
  • Promote data use for performance monitoring, program improvement, and feedback to clinical and management teams
Coordination, Capacity Building, and Support
  • Work closely with clinicians, nurse mentors, and M&E staff to promote accurate data recording and reporting
  • Provide on‑the‑job support to clinical staff on proper completion of registers and reporting tools
  • Participate in routine data review meetings, facility performance reviews, and supervisory visits
  • Assist during data verification exercises, internal reviews, and external audits as required
Other Duties
  • Attend refresher trainings, workshops, and meetings organised by the Ministry of Health (MOH), PIH Lesotho, or partners
  • Comply with hospital policies, project guidelines, and national health information management standards
  • Uphold professional conduct, teamwork, and accountability
  • Perform any other official duties as assigned

Minimum Requirements

  • Diploma in Statistics or a related field
  • At least three (3) years’ experience in data entry
  • Being a local resident is an added advantage
