Under the Global Fund Grant Cycle 7 (GC7)
Phelisanang Bophelong HIV/AIDS Network, as a Sub-Recipient (SR) of the Ministry of Finance and Development Planning – Project Management Unit (PMU), under the Global Fund GC7 grant, will be implementing community-based TB activities in collaboration with the National TB Program (NTP) and District Health Management Teams (DHMTs).
The project aims to strengthen community TB case finding, treatment adherence, and linkage to care through community-led interventions.
PB therefore invites applications for the position of:
POSITION: Community TB Change Agent
NUMBER OF POSITIONS: Eight (8)
The Community TB Change Agent will enhance TB case detection and promote community awareness through outreach, education, TB screening, sputum collection support, and linkage to care. They will support activities in the catchment areas of the health facilities listed below.

KEY ROLES AND RESPONSIBILITIES
Under the supervision of the Community TB Clinical Nurse and in close collaboration with Village Health Workers (VHWs) and community partners, the Community TB Change Agent will:
- Conduct community sensitization and awareness sessions on TB prevention, signs, symptoms, and available services.
- Disseminate key TB health promotion messages through dialogues, household visits and community gatherings.
- Screen individuals for TB symptoms in households and community settings.
- Identify and refer presumptive TB cases to the nearest health facility.
- Support sputum sample collection and ensure timely delivery to facilities or designated points.
- Document and report all screenings and referrals using standardized tools.
- Distribute HIV self-testing (HIVST) kits and link eligible clients to prevention and treatment services.
- Support integration of TB and HIV services at community level.
- Work closely with VHWs to strengthen TB referral networks.
- Support contact tracing and follow-up of TB clients.
- Assist in distributing IEC materials during outreach sessions.
- Perform any other duties assigned by the supervisor relevant to the program.
MINIMUM QUALIFICATIONS, EXPERIENCE, SKILLS & COMPETENCIES
- Minimum COSC/LGCSE or equivalent.
- At least 1 year of experience in community TB programs.
- Certificate in community health or HIV/TB awareness.
- Strong ability to mobilize and sensitize communities.
- Competence in TB screening, sputum collection, and referrals.
- Understanding of Infection Prevention & Control (IPC).
- Ability to record and report data accurately using provided tools.
- Experience in community data collection and client follow-up.
- Reliable, self-motivated, and able to work with minimal supervision.
- Willingness to travel within communities for outreach activities.
