The Global Fund Grant Cycle 7 (GC7) is implementing community-based TB activities in collaboration with the National TB Program (NTP) and District Health Management Teams (DHMTs).

The project aims to strengthen community TB case finding, treatment adherence, and linkage to care through community-led interventions. PB invites applications from qualified individuals to support community activities in Berea District.


Position: Community TB Change Agents

Number of Positions: Eight (8)

The purpose of the Community TB Change Agent is to enhance TB case detection and promote community awareness through targeted outreach, health education, TB screening, sputum collection support, and linkage to care and treatment services.

They will operate within the catchment areas of the following health facilities:

Key Roles and Responsibilities

Under the supervision of the Community TB Clinical Nurse and in close coordination with Village Health Workers (VHWs) and other partners, the Community TB Change Agents will:

  • Conduct community sensitization and awareness sessions on TB prevention, signs and symptoms, and available services.
  • Disseminate key TB health promotion messages through dialogues, household visits, and community gatherings.
  • Conduct screening for TB symptoms in households and community settings.
  • Identify and refer presumptive TB cases to the nearest health facility.
  • Support sputum sample collection and ensure timely delivery to health facilities or designated collection points.
  • Document and report all screening and referral activities using standardized tools.
  • Distribute HIV self-testing (HIVST) kits and refer eligible clients to prevention and treatment services.
  • Support integration of TB and HIV services at community level.
  • Work closely with Village Health Workers and community structures to strengthen TB referral networks.
  • Support contact tracing and follow-up of TB clients.
  • Assist in the distribution of IEC materials during outreach sessions.
  • Perform other duties relevant to the program as assigned by the supervisor.

Minimum Qualifications, Work Experience, Skills and Competencies

  • Minimum of COSC/LGCSE or equivalent.
  • At least 1 year of prior experience in community TB programs.
  • Certificate in Community Health, HIV/TB Awareness or related area.
  • Strong ability to mobilize and sensitize communities on TB and related health issues.
  • Competence in conducting TB screening, sputum collection, and referrals.
  • Basic understanding of Infection Prevention and Control (IPC) measures.
  • Ability to record, report, and submit data accurately using provided tools (registers, referral forms, etc.).
  • Experience in community-based data collection, client follow-up, and health promotion.
  • Reliable, self-motivated, and able to work with minimal supervision.
  • Willingness to travel within communities and support outreach activities.
