Phelisanang Bophelong HIV/AIDS Network, as a Sub-Recipient (SR) under the Ministry of Finance – Project Management Unit (MOF-PMU) for the Global Fund Grant Cycle 7 (GC7), is implementing community-based TB activities in collaboration with the National TB Program (NTP) and District Health Management Teams (DHMTs) and the project is expected to commence on the 𝟎𝟏𝐬𝐭 𝐍𝐨𝐯𝐞𝐦𝐛𝐞𝐫 𝟐𝟎𝟐𝟓 and will be ending on the 𝟑𝟏𝐬𝐭 𝐌𝐚𝐫𝐜𝐡 𝟐𝟎𝟐𝟕.
The project aims to strengthen community TB case finding, treatment adherence, and linkage to care through community-led interventions. PB invites applications from qualified individuals to support community activities in 𝗠𝗼𝗸𝗵𝗼𝘁𝗹𝗼𝗻𝗴 𝗱𝗶𝘀𝘁𝗿𝗶𝗰𝘁:
𝗣𝗢𝗦𝗜𝗧𝗜𝗢𝗡: Community TB Change Agents – 11 positions
The purpose of the Community TB Change Agent is to enhance TB case detection and promote community awareness through targeted outreach, health education, TB screening, support sputum collection, and linkage to care and treatment services.

𝐊𝐄𝐘 𝐑𝐎𝐋𝐄𝐒 𝐀𝐍𝐃 𝐑𝐄𝐒𝐏𝐎𝐍𝐒𝐈𝐁𝐈𝐋𝐈𝐓𝐈𝐄𝐒
Under the supervision of the Community TB Clinical Nurse, Health Facility and in close coordination with the Village Health Workers (VHWs) and other partners, the Community TB Change Agent will perform the following duties in the catchment of the health facilities:
1. Conduct community sensitization and awareness sessions on TB prevention, signs and symptoms, and available services.
2. Disseminate key TB health promotion messages through dialogues, household visits, and community gatherings.
3. Conduct screening for TB symptoms in households and community settings.
4. Identify and refer presumptive TB cases to the nearest health facility
5. Support sputum sample collection and ensure timely delivery to health facilities or designated collection points.
6. Document and report all screening and referral activities using standardized tools.
7. Distribute HIV self-testing (HIVST) kits to eligible individuals in the community and refer eligible clients to prevention and treatment services.
8. Support integration of TB and HIV services at community level.
9. Work closely with Village Health Workers and community structures to strengthen TB referral networks.
10. Support contact tracing and follow-up of TB clients.
11. Assist in distribution of IEC materials during outreach sessions.
12. Perform other duties relevant to the program as assigned by the supervisor.
𝐌𝐈𝐍𝐈𝐌𝐔𝐌 𝐐𝐔𝐀𝐋𝐈𝐅𝐈𝐂𝐀𝐓𝐈𝐎𝐍𝐒, 𝐖𝐎𝐑𝐊 𝐄𝐗𝐏𝐄𝐑𝐈𝐄𝐍𝐂𝐄, 𝐒𝐊𝐈𝐋𝐋𝐒 𝐀𝐍𝐃 𝐂𝐎𝐌𝐏𝐄𝐓𝐄𝐍𝐂𝐈𝐄𝐒:
1. Minimum of COSC/LGCSE or equivalent.
2. At least 1-year prior experience in community TB programs.
3. Certificate in community health, HIV/TB awareness.
4. Strong ability to mobilize and sensitize communities on TB and related health issues.
5. Competence in conducting TB screening, sputum collection and referrals.
6. Basic understanding of infection prevention and control (IPC) measures.
7. Ability to record, report and submit data accurately using provided tools (registers, referral forms etc).
8. Experience in community-based data collection, follow-up of referred clients, and health promotion activities.
9. Reliable, self-motivated and able to work with minimal supervision.
10. Willingness to travel within communities and support outreach activities.
