TITLE: Community Referrals and Linkages agents

SUMMARY: CID-LINK
Population Services International (PSI) implements community testing services in five lowlands districts. It uses innovative strategies to support MOH to achieve targeted testing to men and women aged 15- 49 years under CID link project. CID-LINK uses innovative strategies to provide confidential, community-based HIV testing and services (HTS) to men and women aged 15-49 years as the major target group
This position is based Berea and reports to the Referral and Linkages Coordinator.
ROLES AND RESPONSIBILITIES
Duties include but are not limited to:
Assist HIV patients in the community and clinic environment to understand, manage and cope with their chronic illness after a positive test.
Explains the scope and importance of the Community Link program to potential clients.
Obtains patient consent for participating in the linkage program (follow-up services).
Collects and records patient contact information on program forms.
Assist client to select user friendly linkage facility and then make required arrangements for the client in accordance with project standard operating procedures.
Follow-up with clients to help ensure linkages to care and treatment and to help clients overcome barriers that prevent such linkages.
Motivate, educate and help patients to live healthy and positively.
Keep and maintain referral and linkage guidelines/protocol for consultation on his/her daily work.
Completes all linkage program forms and registers
Compile monthly reports for referral and linkage, submit them to supervisor.
Collect vital information useful for strengthening the referral and linkage program and share them with relevant staff.
Conduct himself/herself professionally, maintain confidentiality and maturity at all times.
Undertake any other reasonable duties as assigned by the Linkages Supervisor.
EDUCATIONAL QUALIFICATIONS AND WORK EXPERIENCE:
COSC
Previous experience in dealing with HIV issues will be an added advantage
Experienced with HIV testing and counseling programs an advantage

Must be of sober habits
Must be able to communicate in English and
fluency in Sesotho
Willingness to work flexible and evening
hours as required
